Housing First Case Manager

WHO IS A HOUSING FIRST CASE MANAGER?

The Housing First Case Manager is responsible for connecting with clients that are homeless or at risk of homelessness and providing one-on-one support for client-defined goals that include: securing and maintaining housing, accessing income supports, health referral and advocacy, social connection, life skills development, employment and education support. Working with program participants and community partners, the CMHA Kelowna Case Manager will support individuals vulnerable to homelessness in finding permanent housing and maintaining their tenancy, knowing their rights and responsibilities as a tenant, as well as being a good neighbour. This includes individuals in shelter, and those currently in hotels in the community. The Case Manager will develop transition plans and connect individuals to appropriate community resources and move them into different streams of service as needed.

WHAT DOES A HOUSING FIRST CASE MANAGER DO?

  • Develop relationships with landlords and provide informal landlord/tenant mediation when required.
  • Keep current on rental vacancies in the
  • Assist people who are in need of support services to find and maintain their
  • Foster and maintain excellent collaborative relationships with health and social service professionals addressing clients’ outstanding
  • Connect clients to income supports to which they are entitled: Income Assistance, Persons With Disability (PWD), Canada Pension Plan disability benefits (CPP), GST rebates, income tax rebates, Shelter Aid for Elderly Residents (SAFER) and advocacy with income assistance personnel
  • Liaise with community agencies and businesses to fulfill program needs and ensure a smooth flow of information and
  • Provide appropriate information and referrals to appropriate
  • Provide an ethical, efficient and therapeutic approach to dealing with crisis intervention in a timely
  • Provide safe care to the client in potentially dangerous
  • Prepare referral letters and offer referral information to
  • Maintain files, case notes and daily

YOUR STRENGTHS & EXPERIENCE:

  • Minimum Bachelor’s Degree in a Social Services or related field, or equivalent combination of education, training, and experience;
  • Demonstrated skill in supporting individuals experiencing homelessness and a proven track record in providing quality, evidence-based services;
  • Excellent computer literacy skills;
  • Excellent written and verbal communication skills;
  • Knowledge of health and social service systems with understanding of the complexity of the income assistance system;
  • Demonstrated experience working with people experiencing homelessness, including those with serious mental illness and/or addictions and complex health and social issues;
  • Demonstrated experience providing mediation/conflict resolution;
  • Demonstrated negotiation/communication skills;
  • An ability to manage a flexible work schedule;
  • Must possess a valid driver’s license, reliable vehicle for business purposes and acquire liability insurance;
  • Current first aid certification;
  • Proof of COVID-19 vaccination is

IT Technician

WHO IS AN IT TECHNICIAN?

The IT Technician at CMHA Kelowna is an integral part our operations team and the go to person for all things technical. You will be exposed to all aspects of the organizations technology and use your expertise to resolve hardware, software and network problems. This is a perfect position for an outgoing, customer service focused technical support professional that enjoys training and has a desire to support the organizations IT goals.

This is a full-time position offering 37.5 hours per week, Monday to Friday.

PRIMARY DUTIES & RESPONSIBILITIES:

  • Maintain phone and computer systems and networks to optimize performance
  • Hardware and Software installations – setting up phones, computers and peripheral devices
  • Identify and provide IT support, training, orientation and guidance to all organization users
  • Perform troubleshooting to diagnose and resolve problems
  • Develop and implement file storage and security protocols
  • Identify phone, computer and network equipment needs
  • Support ergonomic designs of workspaces to provide efficient and healthy work environments
  • Develop and maintain records of hardware and software inventory, manage software licensing and follow proper procedures for disposal of electronic assets
  • Assist in the development of electronic systems to support asset management
  • Assist with maintaining information security
  • Maintain accurate records/logs of repairs and maintenance schedule
  • Perform other duties as assigned 

REQUIREMENTS OF AN IDEAL CANDIDATE:

  • Proven working experience as an IT Technician or relevant position
  • Demonstrated diagnostic and problem solving skills
  • Excellent written and verbal communication skills with proven ability to communicate effectively with a diverse range of audiences
  • Demonstrates great customer service skills, listens carefully and exercises patience
  • Outstanding attention to detail, organizational and time-management skills
  • Demonstrated ability to develop options and/or recommendations to resolve issues
  • Good knowledge of internet security and data privacy principles
  • Proven discretion and confidentiality of personal information
  • Sufficient physical strength and ability to lift and handle computer equipment and a valid BC driver’s license
  • Certification as IT Technician will be an advantage
  • Must provide proof of full vaccination

Health Care Worker – Ellis Place

WHO IS A HEALTH CARE WORKER WITH CMHA KELOWNA?

As part of the team within CMHA Kelowna Supportive Housing, the Health Care Worker will promote and maintain the health, safety, independence, comfort and well-being of individuals and families. This position will focus on assisting individuals in activities of daily living with attention to their physical health, hygiene and home health. The Healthcare worker will support in accessing and connecting individuals to meet their physical health goals. The Health Care Worker will provide personal care assistance and services in a caring manner that recognizes and supports the unique needs, abilities and backgrounds of the program participants.

This role is permanent full-time, offering 37.5 hours per week at our Ellis Place location.

WHAT IS A HEALTH CARE WORKER RESPONSIBLE FOR?

  • Recognizes and responds to the client`s specific needs with a particular focus on individuals experiencing mental health and substance use needs.
  • Recognizes the effect of illness, stress and disability on program participants.
  • Uses knowledge of the aging process and the impacts of mental health and substance use as it relates to an individuals physical health and chronic conditions to provide safe competent care.
  • Provides individualized age appropriate client-centered care, with a focus on physical, psychological, social, emotional cognitive, cultural and spiritual support.
  • Demonstrates an ability to plan and implement care based on the client’s health status; physical, emotional, social and spiritual well-being; and to take action in collaboration with the team and community supports.
  • Provides individualized client-centered care to support the client’s requirements for activities of daily living to be met (e.g. hygiene, oral care, nutrition, rest, and mobilization).
  • In accordance with the plan of care assists with specific tasks related to medication administration in accordance with established policy. The Health Care Worker will work closely with local pharmacists, prescribers and care teams to support in the individuals goals related to medication.
  • Supports in advocating, accessing, connecting and engaging with medical professionals in the community as per the established care plan.
  • Provides minor wound care as per treatment plan and in collaboration with community partners
  • Demonstrates an ability to care for individuals with cognitive challenges including dementia and delirium
  • Appropriately communicates information about the program participants needs while respecting confidentiality and legally regulated requirements
  • Recognizes and demonstrates an understanding of the role of the Health Care Worker within the team and with the program participants’ community supports.
  • Recognizes and responds to potential risks such as falls, suicide, and choking. 

YOUR STRENGTHS & EXPERIENCE:

  • A degree or diploma from a recognized Health Care Aide Program/Human Service Work, or an equivalent combination of education, training and experience
  • A minimum of one (1) year of recent job-related experience
  • Knowledge and experience working with marginalized and vulnerable populations, in particular those that have experienced homelessness, including individuals experiencing mental health and/or substance use issues
  • Must be able to clear a Criminal Record Check, Vulnerable Sector
  • Valid Level 1 First Aid with CPR
  • Class 5 driver’s license, clean driver’s abstract, applicable liability insurance, and access to vehicle for work purposes
  • Contingent upon proof of COVID-19 immunization
