Employee Daily Pre-shift Health Check

If you are working at any CMHA Kelowna location you must complete this Health Check each day, prior to your shift. Once the health check is completed and submitted you will receive an email acknowledging that we received your Employee Daily Pre-shift Health Check along with the appropriate instructions.

**NOTE: In order to make the Health Check easier to complete we have removed the response boxes. Please fill in your name, email and your manager’s email and review the list below. Once you have completed the review please follow the directions at the bottom of the screen.






    Have you travelled outside of Canada within the last 14 days?

    Have you been identified by Public Health as a close contact of someone with COVID-19?

    Have you been told to isolate by Public Health?

    Are you displaying any of the following new or worsening symptoms:

    • Fever or chills
    • Cough
    • Loss of sense of smell or taste
    • Difficulty breathing
    • Sore throat
    • Loss of appetite
    • Extreme fatigue or tiredness
    • Headache
    • Body aches
    • Nausea or vomiting
    • Diarrhea

    If you have answered YES to ANY of the symptoms or questions above, do not go into work, call 811 for direction and call your manager.
