Case Management
(Housing First, Rapid Rehousing and Market Rental Supportive Housing)
Our team provides recovery-oriented and harm reduction focused case management and housing support to complex and vulnerable individuals. Caseloads are small (10-25 max) in order to provide quality services to our community’s most at-risk and marginalized individuals. Referrals to our case management team are made by the Access Navigator, after an initial needs assessment has been completed. To refer yourself or someone you are supporting to our case management team, please submit a Supportive Housing Registration (SHR) Service Form.
